The trauma that current methods of nasotracheal intubation can cause to the patient’s nasal passage is extensive, and the complexity of this technique for medical professionals can be frustrating, particularly in emergency trauma situations. The solution was the development of safer, simpler and more secure methods of intubation. Dr Vernon Vivian was shown how to insert the end of an endotracheal tube into the flute of a red rubber catheter to reduce the complications and make nasal passage easier. The drawback was the thick wall of the catheter adding to the external diameter of the tube which can add to trauma.

The solution was the OBTURATING INTRODUCER FOR A NASAL ENDOTRACHEAL TUBE.
It then became apparent that if you could secure the airway by the oral route, which all anaesthetists are accustomed to, then the nasal airway could be secured in a safe, controlled manner. Using the introducer and a further length of flexible reinforced airway tubing, the method of retrograde nasal intubation allowed the oral airway to be changed to a nasal airway.
Consequently, the NASAL LARYNGEAL MASK and the INTERCHAGEABLE ORAL/NASAL ENDOTRACHEAL TUBE were developed.
