Using an app and a selection of hardware, we are able to efficiently monitor at-risk groups outside the clinical setting during more constrained periods. The user-friendly design allows for easy adoption and straight-forward understanding of each patient`s health status. The pulse oximeter and digital thermometer capture data whilst worn by the patient and transmit the information to the clinician`s dashDoard as well as ;o the patient`s Electronic Health Record (EHR) via the Hex Remote Monitoring application. The solution offers full interoperability with EHRs and coding records, and the server is HL7-FHIR compliant.
