Microorganisms produce nano-sized lipid bilayer particles called microbial Extracellular Vesicles or EVs.

Some microbial EVs play a key role in regulating human immune function and metabolic homeostasis.

MD Healthcare is developing first-in-class smart drugs using Lactobacillus-derived EVs that can be orally administered and penetrate the blood-brain barrier to target neurons in the brain.

Neurons can be damaged by unregulated protein misfolding and aggregation, leading to disorders such as Autism Spectrum Disorder, Alzheimer's Disease, and Parkinson's Disease.

Progression of these brain disorders is associated with uncontrolled ER stress due to the build-up of misfolded proteins within the endoplasmic reticulum or ER.

Inside the cell, EVs are carried to the ER Lumen by endosomes. EVs release metabolites that act on the ER membrane, activating GPCR.

This enhances signaling cascades, such as AMPK and CHOP signaling pathways activated by IRE-1 and PERK, thereby augmenting an adaptive Unfolded Protein Response called UPR.

UPR normalizes protein folding, which relieves ER stress, increasing cell survival and improving anti-inflammatory responses, ultimately enhancing neurogenesis and interneural integrity.

As a promising candidate therapy, microbial EVs offer new possibilities in the treatment of brain disorders.
